What is a Community Health Worker?

A Community Health Worker is a trained member of your community who helps you access the health care that is available to you. They know the clinics, the paperwork, and the local resources, and they speak with you as a neighbor rather than a case file.

  • Understand your benefits and paperwork

  • Find a doctor or clinical and get to your appointment

  • Connect to food, housing, and other local resources

  • Follow up after a hospital or clinical visit
