Vista Care LLC provides Community Health Worker services to Medi-Cal members across Kern County. We are local, we are covered by your plan, and we work on the part of health care that happens after you leave the clinic.

Our Mission
To make sure every Medi-Cal member in Kern County can actually use the care they are entitled to.
Coverage on paper is not the same as care received. Across Kern County, people who are fully insured still miss appointments they need, leave the clinic unsure what they were told, and go without resources they qualify for, because the distance between a benefit and a person is made of phone trees, forms, transportation and time that most households do not have.
Vista Care exists to close that distance. Our Community Health Workers come from the neighborhoods they serve, and they stay with a member through the whole of it — explaining, navigating, advocating and following up until the appointment happens, the resource comes through and the question gets answered.
We measure our work not by referrals sent but by care actually received, and we do it one person at a time, at no cost to the people we serve.

The Work We Do
Our Community Health Workers meet members where they already are — at clinics, at community events, at home and on the phone. Four services carry most of that work.

Screening and Assessment — health and household needs, start to finish
Support and Advocacy — someone with you when a system is hard to move

LEADERSHIP
Sheila Bagsby
CEO and Director of CHW Services
Sheila leads Vista Care's Community Health Worker program and works directly with clinics, health plans and community organizations across Kern County to get members connected to a CHW.
She brings that work a residential operator's understanding of what recovery actually requires. Sheila also operates Buena Vista Manor ARF Inc in Bakersfield, an adult residential facility serving adults 18 and older 24 hours a day, 7 days a week in a safe, stable, recovery-oriented and supportive living environment, with assistance for self-help skills, physical challenges and mobility, and support for behavioral challenges.
Buena Vista Manor was built on the conviction that housing is a first step to recovery, and that a home-like family atmosphere prevents the acute illness and decompensation that push people into more restrictive levels of care. Years of running a home on that principle — meals, medications, transportation, supervision and recreation, every day without a gap — are what shape how Sheila builds Vista Care's CHW program: meet people where they live, remove the barrier in front of them, and stay until it is resolved.
