COMMUNITY HEALTH WORKER SERVICES

A neighbor who knows the system

A Community Health Worker is a trained member of your community who helps you use the health care you already have. They know the clinics, the paperwork and the local resources, and they speak with you as a neighbor rather than a case file.

COVERED BY MEDI-CAL AT NO COST TO YOU

What a CHW does

EXPLAINS THINGS PLAINLY

Your benefits, your paperwork and what the doctor actually said.

GETS YOU TO CARE

Finding a provider, booking the visit and arranging the ride.

STAYS WITH YOU

Follow-up after a hospital or clinic visit, for as long as you need.

CONNECTS RESOURCES

Food, housing, utilities and other local help when you need it.

What a CHW is not

A Community Health Worker works alongside your medical team, not in place of it.

  • Understand your benefits and paperwork

  • They do not replace your doctor, clinic or health plan

  • They do not decide your benefits or report on your household

  • Working with one costs you nothing and is entirely your choice

Why it Helps

Most of what makes care hard happens outside the exam room — the call that goes unanswered, the ride that falls through, the form nobody explained. A CHW handles that part, so the care you are already entitled to actually reaches you.
