What a Community Health Worker does for you

HEALTH EDUCATION

Your condition, your medications, and your options, explained in plain language and at your pace. We go over what the doctor said after the visit, so it makes sense when you get home.

  • Understanding a new diagnosis

  • Managing your diabetes, blood pressure, and chronic conditions day to day

  • How and when to take your medications

HEALTH NAVIGATION

Appointments, referrals, transportation, and the paperwork in between. If a system is confusing or a phone call goes nowhere, your CHW works with you.

  • Finding a doctor, dentist, or specialist who takes your plan

  • Scheduling and getting your appointment

  • Following up after a hospital stay or clinic visit

SCREENING AND ASSESSMENT

A short conversation about your health and your household, so we know what you actually need. Nothing is required of you, and nothing you share affects your benefits.

  • Health and wellness check-in

  • Food, housing, utilities and transportation needs

  • Referrals to local resources that fit your situation

Someone in the room with you when a system is hard to move. Your CHW can sit in on a visit, speak up on your behalf, and stay with you until the issue is resolved.

SUPPORT AND ADVOCACY

  • Support at appointments and health plan calls

  • Help with denials, coverage questions, and paperwork

  • Ongoing check-ins for as long as you need them
